Dr. Chinyere Nwafor GP Sheffield leads the dedicated team at Carrfield Medical Centre, a proud part of the Heeley Plus Primary Care Network. We focus on personalised, patient-first care for individuals and families across Sheffield. Your health, comfort, and peace of mind are always our top priorities.
Booking an appointment is simple and fast. Use our online form to contact the clinical team directly. However, if you have accessibility needs, call us at 0114 258 4724. For all others, our reception may kindly guide you to use the online form.
Please do not use this form in emergency situations. In such cases, always call 999 or NHS 111 right away.
If you’re unwell and need urgent attention, contact us as soon as possible. A doctor will speak with you and offer advice. If appropriate, a same-day appointment will be arranged without delay.
For routine matters like vaccinations or screenings, you may book or amend appointments by phone. Additionally, if you scheduled online via a self-booking link, you can make changes up to 12 hours in advance.
Conversely, if you’re within 12 hours of your appointment, please call us directly for support.
We provide home visits for patients who are housebound or severely unwell. For this reason, please contact us before 10:30 AM to allow the GP team to organise the day’s priorities efficiently.
Patients eligible for home visits include those with terminal illness, frailty, or conditions that worsen with travel. However, if you are able to come into the clinic safely, this is always preferred for quicker access to care.
When the practice is closed, you can reach NHS 111 for guidance. This free, 24/7 service helps with non-life-threatening issues. However, in life-threatening emergencies, dial 999 immediately.
Critical emergencies include:
Chest pain or suspected heart attack
Breathing difficulties
Stroke symptoms
Head trauma
Loss of consciousness
Stay calm and help where possible. However, do not give food or drink unless told to by professionals.
A&E is appropriate for serious injuries or urgent health problems. For instance, deep wounds, fractures, or uncontrolled bleeding may require emergency care. On the other hand, minor issues are best handled by your GP or NHS 111.
Visit the NHS website for reliable information on symptoms, treatments, and health conditions.
