Who Can Request Dental Images

Patients aged 21 years and above can request dental images directly.
Patients below 21 years need parental or legal guardian consent.

How to Request Dental Images

You can request dental images via email. Please send your completed request form and all required documents to dro@ndcs.com.sg. Once we receive your request, we will register it and provide payment instructions.

Required Documents for Dental Image Requests

You must complete the Dental Images Request Form and include the following:

• Proof of identity (NRIC or Passport)

• Completed consent form if under 21

• Any other documents listed in the request form

Charges for Dental Images

The fees are simple and GST inclusive:

• Digital X-ray: $30 for 1 to 5 latest images

• CBCT Scan CD Copy: $25 per scan

Delivery Methods for Dental Images

We provide secure and efficient delivery:

• Digital dental images will be sent via encrypted PDF to your email.

• CBCT scans in CD format must be collected in person at our office.
