National Heart Centre Singapore Medical Reports help patients access their health information efficiently. Patients can request medical reports, duplicate discharge summaries, laboratory results, or X-ray reports. National Heart Centre Singapore Medical Reports provide accurate and timely processing for patients and authorized third parties. By using this service, patients receive their reports securely and quickly.
Patients aged 21 years and above can request their own medical reports. Minors may apply with a parent or legal guardian’s consent. In cases of deceased or mentally incompetent patients, authorized representatives or next-of-kin must provide written consent. This ensures that all requests follow legal and ethical standards.
Patients can submit requests online through the HealthHub portal. First, log in using SingPass. Then, navigate to ‘Records’ and select ‘Medical Reports/Certificates Request.’ Next, choose the type of report needed. After submitting the online form, the system sends payment instructions directly to the patient. This method is convenient, secure, and saves patients a trip to the hospital.
Patients without SingPass can submit a hardcopy form. Complete the ‘Application and Consent for Release of Medical Information’ form. Minors require parental or guardian consent forms. Attach identification documents, such as NRIC or passport, and insurance forms if needed. Submit the application via email to medicalreport@nhcs.com.sg or via FormSG. Incomplete submissions delay processing, so double-check all documents before sending.
Ordinary medical reports and insurance form completion by doctors cost $121.00. Doctor’s memos, which provide brief diagnostic statements without detailed explanations, cost $12.00. Duplicate copies of discharge summaries, lab results, or X-ray reports also cost $12.00. Patients must pay once the application is processed because payments are non-refundable.
National Heart Centre Singapore Medical Reports usually take 4–6 weeks to process. Delays may occur if patients have upcoming appointments, are currently admitted, request reports from multiple departments, or if doctors are on leave. Therefore, patients should submit requests early to avoid delays.
Patients can submit hospitalisation or critical illness insurance forms for doctor completion only after discharge. Confirm the required form with your insurance company or agent. Once processed, the completed form will be returned promptly.
For enquiries, email medicalreport@nhcs.com.sg or call 67042322. The unit usually responds within three working days. Staff operate Monday to Friday, 8.00am–5.00pm, and close on weekends and public holidays.
National Heart Centre Singapore Medical Reports make healthcare simple. Patients can manage requests quickly, access reports safely, and keep their health information organized. Additionally, using these services helps patients stay informed about their heart health and treatment history.
