She gained practical experience during her internship at Chris Hani Baragwanath Hospital and as a community service medical officer at Helen Joseph Hospital. Her work included orthopaedics, intensive care, and paediatric care.

She completed a six-month senior house officer post in paediatrics at Chris Hani Baragwanath Hospital. In 2004, Dr Spiegal qualified in Diagnostic Radiology through the College of Radiologists of South Africa. Since then, she has advanced to become a senior consultant and acting clinical head in radiology departments before joining Dr L Sulman and Partners in 2009 as a specialist radiologist.
