Who can request dental images?

• Patients aged 21 years and above may make requests themselves.

• Patients under 21 can request with consent from a parent or legal guardian.

How to request your dental images

1. By Email
   Email your completed request form and required documents to dro@ndcs.com.sg.

2. Fill out the Dental Images Request Form
   Download the form and complete all fields.
   Include the supporting documents listed in instructions 1 & 2 on the form.

3. Submission and Verification
   Once we receive the form and documents, we register your request.
   Then we will send payment instructions. We only proceed after verification.

What are the charges?

Delivery modes

• Digital format: You will receive an encrypted PDF via email.

• CBCT scans on CD: You must collect the CD in person.
  We process your application only after verification, payment, and receipt of fully completed forms.
