The National Dental Centre Singapore Dental Records Office handles requests for dental reports, images, and personal dental documents. Patients and authorised individuals can access these reports for insurance claims, work-related compensation, or personal reference.
Who Can Request Dental Records
At the National Dental Centre Singapore Dental Records Office, patients over the age of 21 may request their own records. For patients under 21, a parent or legal guardian must provide consent. In special cases, such as mental incapacity or death, the authorised representative or next-of-kin may submit the request.
Third parties may also request records, but only with the patient’s consent. This process ensures your dental information is protected while still accessible when needed.
You can submit a request through HealthHub with SingPass, or email the Dental Records Office directly. Available record types include:
Ordinary dental reports
Specialist reports
Insurance form completion
Work injury forms
Simple insurance documents
Memos
Record duplications
If you need a specific document not listed, the team at the National Dental Centre Singapore Dental Records Office will assist by email.
The cost of services varies. Ordinary dental reports cost $121, while specialist reports are priced at $222. Insurance form completion and work injury reports typically cost $121. Basic memos and simple insurance forms range from $12 to $25.
All fees include 9% GST. Payment methods include PayNow or HealthHub (credit card). Once the request is verified and payment is made, processing begins.
Dental record requests typically take four to six weeks. The timeline begins once all documents are completed, signed, and payment is received.
