If you prefer, you can visit Parkway East Hospital’s Medical Records Office for assistance. Before contacting the office, have the following ready:

• Application and Consent for Release of Health Records

• A copy of the patient’s and authorised requestor’s NRIC or passport

• For patients below 21 years old, consent from a parent or legal guardian

• For deceased patients or those lacking mental capacity, consent from a legal representative and a copy of the death certificate if required
