Who Can Request Medical Records

Patients aged 21 and above can request their medical reports. Minors may request medical records with parental or legal guardian consent. Third parties may request records with patient authorization. For deceased or mentally incompetent patients, the authorized representative or next-of-kin must provide written consent.

Types of Medical Reports Available

We provide various medical reports to suit different needs:

• Ordinary Medical Report: Includes comprehensive details about medical conditions and treatment, commonly requested for personal or insurance purposes.

• Specialist Medical Report: Offers doctors’ professional opinions and prognosis, often requiring assessments at the Specialist Outpatient Clinic.

• Psychiatric Reports: Specialized reports completed by the Psychiatry Department.

• Workman Compensation Reports: For evaluating work-related injuries and disability duration.

• Deputyship Medical Report/Form 61: For activating LPA or appointing a deputy for individuals lacking mental capacity.

• Duplication Services: Certified copies of medical certificates, referral letters, investigation results, radiological images, and discharge summaries.

How to Apply

Applications for medical records must include completed forms and supporting documents. Forms A is for the application and consent. Form B applies to deceased or mentally incompetent patients. Form C is for credit card authorization if needed. Incorrect or incomplete forms may delay processing.

Fees for Medical Reports

Medical reports have a structured fee schedule:

• Ordinary and insurance reports: $121

• Specialist reports: $222

• Psychiatric specialist reports: $480

• Deputyship reports: $222 (or $480 for psychiatric)

• Copies of certificates or reports: $12-$78 depending on type

Please allow 4-6 weeks for processing. Delays may occur if patients are warded, doctors are away, or requests involve multiple departments.
