Complications and Recurrence of Bartholin’s Cyst

Common complications include:

  1. Infection and abscess formation
    • A simple cyst can quickly become infected, turning into a painful abscess filled with pus.
    • Infection may spread to nearby tissues if untreated.
  2. Chronic pain or discomfort
    • A large cyst or repeated abscess can cause ongoing pain, especially during walking, sitting, or sex.
  3. Difficulty with intimacy
    • Pain or anxiety around intercourse may arise if the cyst returns or causes scarring.
  4. Scarring and tissue changes
    • Repeated infections or surgeries may result in scarring or narrowing of the vaginal opening (stenosis), affecting sexual function or childbirth.
  5. Bartholin’s gland carcinoma (rare)
    • Though very rare, especially under the age of 40, a cancerous tumour can mimic a cyst. This is why any new lump in older women should be carefully assessed and biopsied if necessary.

Why do some women get recurring cysts?

  • Incomplete drainage of the original cyst
  • Continued exposure to irritants or infection
  • Narrow ducts or scarring from previous procedures
  • Untreated underlying infections, including STIs

Preventing recurrence:

  • Complete full antibiotic courses when prescribed
  • Use sitz baths regularly during recovery
  • Practice safe sex to reduce STI risk
  • Keep the area clean and dry
  • Wear cotton underwear and avoid tight clothing

In South Africa, access to follow-up care may be limited, making recurrence more likely in women who are discharged without further monitoring. Education on home care and follow-up is crucial.

If cysts return frequently, more permanent solutions like marsupialisation or gland removal should be considered. These options offer long-term relief and restore comfort and confidence.
